Theatre Eligibility/Registration Form

This one form is used for District and Sectional Theatre festivals. It will be sent on to the next level based on the evaluators' recommendations. Do not skip any items.

Remember, you are required to show proof of rights to produce the script below. This should be in the form of:

A receipt for payment to the publisher for performing the script or cutting of the script;
A letter from owner of the copyright stating your right to produce the script; or
Your letter of inquiry as to the owner of copyright in the event that the owner's name is not known.

Send proof of performance rights to your district chair before the District festival.

A reminder - Theatre Handbook, "Guidelines for Theatre," 8-F: "Upon submitting an Eligibility-Registration form for any WHSFA festival, the member school becomes obligated to pay the registration fees whether or not the school actually attended the festival.

Each level host will provide you with a request sheet for furniture and lighting needs as well as a diagram of the performing area. Please complete those sheets and send directly to the host school.

At the bottom of the form, you will enter students' names in the cast and crew sections. Type each character and the name of the student who plays him/her (one per line) or type each tech position (such as Stage Manager) and the name of the student holding that position (one per line). However many students you enter today is the maximum you may have at any level of participation; substitutions, but not additions may be made.
