District/Section Financial Report

Use this form to summarize income and spending for your district or section account. Do NOT use dollar signs when reporting amounts; just type numbers.
  • This should be the bank statement of the last month with activity, or June. This alternatively may be emailed to office@whsfa.org
    Accepted file types: pdf, doc, docx, pages, rtf, jpg, jpeg, gif, png.
  • Please provide also upload a complete itemized transaction list, which can be prepared in Microsoft Excel, a PDF output from some other program, or scanned from checkbook pages. By law, we specifically need to document any individuals paid directly for their time (honoraria). This alternatively may be emailed to office@whsfa.org
